At Nagoya Eye Clinic, vitreous surgery is performed by retina specialists Dr. Kaga and Dr. Yoshida. The extensive experience and high level of technical skill of both doctors, combined with cutting-edge surgical equipment, ensures safe and reliable surgery. In addition, with consideration for the lifestyle of our patients, we have a system in place that allows for same-day surgery.

About floaters

Inside the eye there is a transparent jelly-like substance called the vitreous. This vitreous can become cloudy for some reason. In many cases, patients will experience symptoms such as "floaters (black shadows flying)". Most floaters are called physiological floaters and do not pose a health problem, but if you wish to improve the symptoms, there are treatments such as neodymium YAG laser and vitreous surgery, depending on the situation.
In addition, some floaters are not actually physiological floaters, but are a sign of a serious illness, or a disease that requires treatment may have already developed. Therefore, we recommend that you do not self-diagnose, but see a doctor if the symptoms persist.

What is vitreous surgery?

This is a surgery that removes the vitreous (a jelly-like tissue inside the eye) and directly addresses and treats the cause of the obstruction of function.
A special, very thin instrument called a vitreous cutter is inserted into the eye through the white of the eye, and any bleeding or cloudy vitreous is excised and removed by suction. Depending on the illness, a membrane may form on the retina, which may be adversely affecting the retina, so a special instrument is used to peel it off and remove it. For weak or holey areas of the retina, treatment is performed using a laser beam to coagulate and adhere the retina. If the retina has detached, gas is injected and the gas's surface tension is used to return the retina to its original position.

Vitrectomy is mainly performed as follows:
Symptoms and illnesses
Eligible

  • ● Floaters: A symptom in which black dots or threads are seen in front of the eyes
  • ● Preretinal membrane (premacular membrane): You may notice distortion of vision or decreased vision.
  • Macular hole: You may feel distortion in your central vision or see a black hole in your vision.
  • ● Diabetic retinopathy: Diabetes causes bleeding from the retina, resulting in loss of vision, and the proliferative membrane can cause retinal detachment, resulting in loss of vision.
  • ● Retinal detachment: When the retina detaches, vision suddenly narrows and vision decreases.

About endoscopic vitreous surgery

Nagoya Eye Clinic has introduced vitreous surgery using an ocular endoscope. This technology is highly related to improving visibility during surgery and reducing the risk of postoperative complications, allowing for safe and secure surgery. In addition, there is no pain caused by pressure on the eyeball, reducing postoperative stress.

Features and Advantages of Ophthalmic Endoscopes

01
Improved visibility

Even in cases of vitreous hemorrhage or corneal opacity, the use of an ophthalmoscope allows the surgeon to perform accurate procedures while directly observing the intraocular condition. This dramatically improves the surgeon's ability to make decisions during surgery, which also reduces the risk of complications.

02
Freedom of approach

Endoscopes can be used to view the peripheral retina and small lesions that are difficult to view under a microscope, and are flexible enough to treat cases that were previously difficult to view.

03
By pressure
No pain

The periphery of the eyeball cannot be observed without using an ocular endoscope or applying pressure (pressing the eye with a thin rod-shaped instrument). However, using an endoscope eliminates the need for pressure, eliminating the pain caused by this pressure.

Surgical flow

Surgery time: Approximately 30 minutes

01

You will be given a local anesthetic so you don't feel any pain.

02

Three small holes are made in the white of the eye to allow surgical instruments to be inserted.

03

The cloudy vitreous is slowly removed using a special instrument called a vitreous cutter.

04

Depending on the condition, retinal treatment or laser treatment may be performed.

05

The removed vitreous body is replaced with saline (water similar to body fluids) or medical gas (air), and the internal pressure of the eye is controlled to complete the surgery.

Precautions after surgery

If gas is turned on, you may need to lie face down.
After surgery, be careful not to rub your eyes and be sure to apply any eye drops prescribed to prevent infection.
[Follow-up observation]
After surgery, you will need to visit the hospital regularly, including for dilated eye exams, to closely monitor your progress.

Q&A about Vitrectomy

Is the surgery painful?
Since local anesthesia is used, you will hardly feel any pain.
How much vision will be restored?
The degree of recovery varies depending on the condition, so please consult with your doctor to determine the best course of treatment for your condition.
How soon can I return to my daily life?
Most people are able to return to their normal daily lives within a few days to a week.
Is hospitalization necessary?
In our hospital, the surgery is performed as an outpatient procedure. If you would like to be hospitalized, we can refer you to a hospital.
What will my vision be like after surgery?
Immediately after surgery, your vision may be blurred or dark due to the effects of the surgery. Depending on the condition and individual differences, recovery may take several weeks to several months.

Cutting-edge vitreous surgery equipment

Major features

High-speed resection
The high-speed cutter cuts 20,000 times per minute, reducing traction stress on the retina, allowing smoother removal of the vitreous body and shortening surgery time.
Precise intraocular pressure control
Our unique fluid control technology stabilizes intraocular pressure, helping to reduce stress on the retina during surgery and facilitating recovery after surgery.
LED Light Source System
It provides bright, uniform illumination while reducing heat generation to reduce damage to intraocular tissue. It is a patient-friendly design.
Various gauge sizes available
It is compatible with 23G, 25G, and 27G and can flexibly adapt to a variety of cases, allowing us to provide optimal treatment for each individual patient.
