Presbyopia (farsightedness) Presbyopia (farsightedness)

Signs of eye changes with age
When you notice it, it's time to see a doctor

What is presbyopia?

The ability to focus on objects close to you gradually weakens with age.
This is a natural change that many people experience, and it becomes easier to notice it from around the age of 40.

Do myopic people get presbyopia?
Do myopic people get presbyopia?
A nearsighted person has a point that can be seen with the naked eye nearby, so if you remove your glasses, you can see your hand. Therefore, presbyopia may be difficult to feel, but everyone can develop presbyopia depending on their age.

presbyopia self check

1 or more: Visit an ophthalmologist to measure your near vision (vision at hand),
If necessary, you may want to get some weak reading glasses.
If you force yourself to go without glasses, it may cause eyestrain.

How to correct presbyopia

Around the age of 40, both those who have had good eyesight and those who have had bad eyes will begin to experience difficulty seeing.
Nowadays, there are various correction methods other than reading glasses, but there are various causes of difficulty in seeing, and the correction method that suits your work, hobby, lifestyle, etc. is also different.
We recommend that you first consult an ophthalmologist and consult with a doctor or an orthoptist.

Pilocarpine eye drops

This treatment method uses pilocarpine eye drops, which have a miotic (pupil narrowing) effect, and is expected to have the effect of widening the range of focus (depth of focus), making it easier to see nearby objects, just like when you narrow the aperture of a camera.
Although this varies from person to person, it is thought that a single dose of eye drops can assist with near vision for around six hours.

At our clinic, we self-prescribe two types of pilocarpine, 0.4% and 1.25%, which are the same concentrations as those approved in the U.S. We select the concentration based on the patient's eye condition.

Cost

[Caution] For those who wish to use pilocarpine eye drops to treat presbyopia

● Cannot be used in conjunction with insurance treatment
Presbyopia treatment is not approved in Japan and is a private medical treatment. According to national rules, you cannot receive an "insured consultation" for dry eyes, styes, etc. on the same day, and all consultations and prescriptions must be paid for out of pocket.
● If you have other symptoms, please visit the doctor on another day.
If you would like to be examined for other problems, please come back on another day.

Risks/side effects

Relatively common side effects
Headache, redness, pain when applying eye drops, temporary blurred vision or changes in vision.
Caution due to small pupils
You may find it difficult to see in dark places.
Serious Side Effects
Rarely, retinal tears and retinal detachment have been reported.
Those who cannot use pilocarpine eye drops
  • ・People who are allergic to pilocarpine (Sanpilo)
  • - People with retinal diseases such as retinal tears or retinal detachment

At our clinic, we first conduct an examination to confirm whether the cause of your vision problems is presbyopia before prescribing. We then check the condition of your retina, and when prescribing for the first time, we apply eye drops in the clinic and monitor your progress for about 30 minutes before prescribing.
Depending on the condition of your eyes, we may suggest treatment for cataracts or other conditions (such as dry eyes).

The doctor will make a decision on whether to continue the prescription after evaluating your vision and checking the severity of side effects.

Bifocal contact lenses

Recent advances in bifocal contact lenses have resulted in significant improvements in both natural vision and comfort.
There are more designs that offer a better balance between far, middle and near vision, making it comfortable to see at everyday distances such as when working on a computer or using a smartphone.

特長

  • ● Instant focus on both near and far objects
  • ● No discomfort in the field of vision, it looks natural
  • ● Easy to get used to
  • ● At first, your brain may have trouble focusing and you may feel uncomfortable, but most people will get used to it within a few days to a week.

At our clinic, we offer a wide range of contact lenses, including hard and soft (1-day, 2-week) types, to suit each patient's lifestyle. We will work with you to determine the best choice based on your vision and the occasion in which you will be wearing them.

Recommended for these people

  • ● I don't want to have surgery, but I also don't want to wear glasses if possible.
  • ● Often look at objects at intermediate distances (such as a computer screen)

Reading glasses and progressive glasses

Reading glassesRecommended for
  • ● Do a lot of detailed work, such as reading or sewing.
  • ● I want to try something simple first.
Progressive glassesRecommended for
  • ● I don't want to change my glasses depending on my work or driving.
  • ● I want to enjoy wearing glasses as a fashion statement.

Recently, some eyeglass stores have eyeglass manufacturing technicians (level 1) on staff.Eyeglass craftsmen are nationally certified professionals who take into account the condition of your eyes and your lifestyle, and who not only select the prescription but also the frame and adjust the fitting. This is especially recommended for those considering getting bifocal glasses.

You can find out where your nearest eyeglass store employs eyeglass craftsmen here.

Presbyopia-friendly ICL

While regular ICLs are primarily used to correct myopia and astigmatism, new ICLs have been developed that are also designed to treat presbyopia.
This is collectively calledPresbyopia-friendly ICLThere are two types of them:

1. EVO VivaTM ICL
(EVO+ EDOF ICL)

These refractive lenses are provided by STAAR Surgical. They are designed to maintain natural vision and are suitable for mild to moderate presbyopia. There are no astigmatism models.

2.IPCL Multifocal

These are diffractive lenses provided by Care Group. You can choose the power for near vision, making them easy to match to the degree of presbyopia. Astigmatism can also be corrected.

Comparison of ICLs for presbyopia

Demerit

  • Glare and halos at night are more noticeable compared to conventional ICLs (especially with IPCLs)
  • ・Due to the progression of presbyopia, vision may become more difficult after surgery than it was at the beginning.
  • ・There is a possibility that contrast (clarity) in the distance will decrease.

Adaptive test
Adaptive test
We will conduct a thorough examination to determine whether the patient's eye condition is suitable for Comfort ICL.
Test contents: refraction test, visual acuity test, corneal shape analysis, intraocular pressure test, corneal endothelial cell test, slit lamp microscopy, fundus test, anterior chamber depth test, etc.
Cypresin test
Cypresin test (approximately 1 hours)Discerning Point
In order to prevent overcorrection, we have been particular about cypresin testing and have been thorough since we opened our clinic.
"Cypresin ophthalmic solution" (hereinafter referred to as Cypresin) has a parasympathetic effect that acts on the ciliary muscle that adjusts the focus of the eye, removing excess eye tension and achieving correct refraction (myopia, farsightedness, astigmatism). used to inspect.Cypress resin may cause symptoms such as difficulty focusing and glare for 2 to 3 days after the test, but please understand that this is a necessary condition to avoid overcorrection.
【please note】
It will be difficult to see your hands for a few days and you will feel the glare, so pleasePlease refrain from driving.
*If you have plans to drive, you may come back to the hospital at a later date.

ICL・SMILE
Monovision

Monovision is a method of using a difference in the power of each eye to compensate for the range of vision for near and far objects. There are three types of difference in power: monovision, mini monovision, and micro monovision, and the smaller the difference, the more natural the vision tends to be.

Comparison with ICL and IPCL for presbyopia
  • ● Less susceptible to glare and halos at night
  • ● Less contrast loss
Disadvantages of monovision
  • ● Wear glasses when driving at night or working at close range for long periods of time.
    may be necessary
  • ● You may feel discomfort due to the difference between the left and right sides.
Preoperative simulation
The most important thing about monovision is to let you experience what your vision will be like after surgery.
At our clinic, we prescribe soft contact lenses with the expected postoperative strength before surgery, and have patients try them on in their daily lives for a certain period of time before making a final decision on the degree of difference between the left and right lenses.
