What is ptosis?
What is ptosis?"A condition in which the upper eyelid droops, making it difficult to open the eye, narrowing the field of vision, and making it difficult to see things."Say.
Patients with ptosis unconsciously try to maintain good vision by lifting their eyebrows, widening their eyes, or constantly lifting their chin when looking at things. This chronic tension in the forehead muscles can lead to headaches and stiff shoulders. As symptoms worsen, it can also cause nausea, dizziness, and sympathetic nervous tension. Other causes, such as myasthenia gravis and oculomotor nerve palsy, can also cause double vision (diplopia), so it is important to thoroughly investigate the cause. Ptosis in only one eye is easy to notice because of the external asymmetry, but in cases involving both eyes, it can be more difficult to notice.
Severity of ptosis
-
normal state
The edge of the upper eyelid touches the cornea (black part of the eye)
Almost no charge -
Mild ptosis
The edge of the upper eyelid meets the cornea (iris)
A condition that hangs between the pupils -
moderate droop
Affecting the upper half of the pupil -
strength drop
Covering the lower half of the pupil
Main treatments for ptosis
First, the cause should be investigated, and if it is a systemic disease such as myasthenia gravis, drug treatment will be necessary. The main treatment for ptosis caused by aging or contact lenses is surgery.
After surgery, unnecessary strain is no longer applied, so you can expect to see relief from stiff shoulders and headaches. Vision also becomes brighter, which may also improve your mood. If another disease is the cause, treatment of the underlying disease takes priority.
Eye drop therapy: a new treatment option

Until now, surgery has been the primary treatment for age-related ptosis and ptosis caused by contact lenses.
In 2026, a new treatment option will be added: eye drops (Upnik Mini eye drops 0.1%), allowing patients to choose their treatment method according to their symptoms and preferences.
*Appneque Mini Eye Drops 0.1% are not covered by health insurance and are therefore not covered by insurance.
Treatment methods tailored to the symptoms and causes
The appropriate treatment for ptosis (drooping eyelids) varies depending on the cause and severity.
The ability to lift the eyelids is slightly reduced.
If it is decreasing
(Mild ptosis)
Eye drop treatment (Upnik) may be indicated. This treatment acts on the muscle (Müller's muscle) on the underside of the eyelid, temporarily lifting the eyelid with the help of eye drops.
*The effects are temporary and it is not a fundamental cure.

The power to lift the eyelids
If it is decreasing
(Moderate to severe ptosis)
Surgical treatment is the primary approach. This involves suturing and tightening the weakened muscles to restore their original function and lift the eyelid.
As skin ages
If it is hanging down
(Eyelid skin laxity)
Surgical skin removal is necessary. By removing the excess skin and suturing it, you can expect not only improved vision but also the secondary effect of making your eyes look younger.
*This type of condition will not improve with eye drops.

Precautions after surgery
- ●Typically, the eye patch will be replaced with a small gauze the next day, and the gauze will no longer be needed the day after. The eyelids are prone to swelling, so they may be temporarily swollen or have purple bruising after the surgery, but this will subside in 1-2 weeks.
- ● You cannot take a bath on the day of the surgery. You can take a bath from the neck down starting the day after the surgery, but please refrain from washing your face and hair for about two days.
- ● Alcohol and tobacco can hinder your recovery, so please refrain from them for about a week.
- ●The stitches will be removed 1 to 2 weeks after the procedure.
[Post-surgery checkup]
You will be asked to come in for a follow-up visit the next day, 1-2 weeks after surgery to have the stitches removed, and then again at 1 month and 3 months.
FAQ
- Is ptosis surgery painful?
- The surgery is performed under local anesthesia. If the anesthesia is effective, you will not feel any significant pain during the procedure. If you do feel pain, additional anesthesia can be administered.
- Can I still wear hard lenses?
- Although it is possible to use it, we do not recommend it as there is a high possibility that ptosis will recur or progress.
- Is it okay to exercise after ptosis surgery?
- Please avoid excessive exercise for one month. Swelling will remain for about one to two weeks after surgery.
- I want to have double eyelids...
- This is not cosmetic surgery. We will do our best to meet your wishes, but the surgery is aimed at restoring function.

What is entropion (inverted eyelashes)?
Entropion is a condition in which the eyelids or eyelashes are turned in an abnormal direction, causing the eyelashes to come into contact with the eyeball (cornea or conjunctiva). If this condition continues, the eyelashes will rub against the cornea, causing damage and unpleasant symptoms such as pain, a foreign body sensation, tearing (watery eyes), and bloodshot eyes. If it becomes severe, it can lead to corneal ulcers and decreased vision.

Normal case
-
In case of entropion
The lower eyelid curls inward, causing the eyelashes to touch the cornea
Types of entropion
There are two main types of inversion:
Entropion
(Gankenaihan)
This type involves the eyelid itself turning inwards. The eyelid position becomes abnormal, resulting in the eyelashes turning inwards (towards the eyeball).
- ● Congenital entropion: This condition is mainly seen in infants. It may improve naturally as the child grows.
- ● Acquired entropion: This occurs due to loosening of the orbicularis oculi muscle and sagging skin caused by aging. It is particularly common in elderly people.
eyelash entropion
(It's pointless)
There is nothing wrong with the position of the eyelid, but the eyelashes themselves grow inward (towards the eyeball). This is often a congenital condition, and when you flip your eyelids, you can see that some of the eyelashes are growing inward.
symptom
● Foreign body sensation (rumbling sensation)
● Eye pain
● Increased blinking frequency
Tears won't stop (lacrimation)
● Bloodshot eyes
● Sensitivity to light (photophobia)
In severe cases, there is a risk of corneal injury and infection.
Treatment
Depending on the symptoms, age, and severity, the following measures will be taken:
In children
(congenital)
- ● If the condition is mild and there are few symptoms: The condition may improve naturally as the child grows, so it is common to just monitor the condition over time.
- ● If there are scratches on the cornea or a chronic foreign body sensation, surgery will be considered. If it is the upper eyelid, the buried suture method will be considered, and if it is the lower eyelid, the Hotz method will be considered. For those aged middle school and older, the procedure can often be completed in a short time under local anesthesia, and it can even be done as an outpatient procedure.
For adults and elderly people
(acquired)
- ● If symptoms are mild, artificial tears or eye drops may help relieve the symptoms.
- ● Temporary treatments include plucking the eyelashes or cauterizing them, but the downside is that the condition tends to recur.
- ● Surgery is the most effective fundamental treatment. By adjusting the skin and muscles properly, the eyelids can be restored to their normal position.
Summary
If left untreated, entropion can damage the cornea and potentially affect vision.
Especially in the case of children, even if you think the condition will improve naturally, it is important to have them examined by an ophthalmologist if the symptoms are severe.

Laser treatment
A CO₂ laser is used to vaporize and remove unwanted tissue. This method is characterized by less bleeding than surgery using a scalpel. Redness may occur after the procedure, but this usually subsides within 1-2 months. Also, please avoid exposure to ultraviolet light after the procedure.
