For me, the only thing that is unique is subretinal endoscopic surgery.
This is a procedure that not only allows for direct observation of subretinal lesions that are difficult to observe using normal microscopic surgery, but also creates a stable and complete subretinal space through irrigation and insertion of instruments, making it possible to perform procedures such as resection, coagulation, and transplantation on subretinal lesions.
In our own cases, if the tissue containing the choroidal neovascularization that is the source of bleeding was completely removed, postoperative vitreous injections were not necessary and blindness was prevented in almost all cases. In the future, if retinal pigment epithelium transplantation using regenerative medicine or other methods can prevent retinal dysfunction, I think this will be an even more revolutionary surgery.

Currently, the only treatment for glaucoma is to lower intraocular pressure, and I struggle daily with how to safely lower intraocular pressure. In my daily practice, I perform more cataract surgeries than glaucoma surgeries, but my only goal is to avoid increasing intraocular pressure during and after cataract surgery.
Even after inserting the intraocular lens, we are careful not to leave behind any cortical tissue that can cause inflammation or any viscoelastic material that can directly cause an increase in intraocular pressure. Furthermore, because we want to end the procedure with as low intraocular pressure as possible, we ensure that the eye is fully self-sealing so that tears do not enter the eye even if it is touched or rubbed.

There is a type of ophthalmic surgery called "intrascleral fixation." This is a surgical method to fix an intraocular lens in cases where it cannot be placed in the capsule. I was involved in the development of a lens suitable for this intrascleral fixation. I devised a method to fix this lens using a 27-gauge needle and vitreous forceps to enable flange fixation by Dr. Yamane.
Whether this is the best or not is up for debate, but for me, this is the only surgical technique I have. When performing intrascleral fixation in cases where a capsule cannot be used, I use this technique.

Long tube shunt surgery (Ahmed, Barberd) is effective for refractory glaucoma, but postoperative corneal endothelial damage is a problem.
What makes me unique is that I devised a new method for using an endoscope in this surgery to determine the tube insertion position while directly observing the ciliary sulcus. By determining the insertion position with an endoscope, safe and reliable tube insertion is possible, and my method was well received when I presented it at an academic conference.
There is still room for improvement in this surgery, so we would like to continue working on further improving the surgical procedure.

Glaucoma is the leading cause of blindness in Japan and is a very serious disease. Recent advances in surgery for glaucoma have been remarkable, with new devices such as the PreserFlow Microshunt and Hydrus being developed and put into practical use one after another.
